  • Photo ID-Government official ID preferred. If you do not have a Drivers License or ID we could also accept a social security card AND a non-government ID that has your current address such as a utility bill.
  • Insurance Card- Must present current card at check in.

Loose fitting clothes that are easy to get on/off

No, please remove all jewelry, this includes body piercings.

Yes, you will remove them before going into the OR.

YES! Driver be 18 years or older.

No cabs, uber, buses, etc.

No, but if not present they do need to be available for a phone call for verbal verification.

Only if the patient is a minor. An adult must stay in the building for minors.

For adult patients the adult driver is allowed to leave and come back.

Yes, It's best to have someone with you for at least the first 24 hours after general anesthesia.

Females who have menstrual cycles MUST give a urine sample at the time they are admitted into Pre op. This includes females who are on birth control.

No sample is needed if post menopause for 1 year, or if you have had a hysterectomy, or tubal ligation.

We are a smaller facility, so only 1 guest per patient. This includes parents with minor patient, only 1 is allowed in our lobby.

Please limit the number of people with you. DO NOT bring children.

No. You may be with your child in pre op until it is time for your child to go back to the OR.

You will join your child in recovery once they awake.

NO, nothing to eat or drink after midnight prior to your procedure, this includes gum and mints. The only exception is if you have remaining colon prep to take.

NO! Nothing by mouth to eat or drink or chew on.

Please consult with your physician on acceptable medication that may be taken the day of your procedure.


If on heart, blood pressure or seizure medication we need to know if you have had a recent cardiac event. You may take these the day of the procedure with a very small sip of water.


If on blood thinner or diabetic medication refer to your prescribing physician or surgeon regarding dosing to take. USUALLY they have you hold off on blood thinner for 1 week prior and oral diabetic the morning of surgery.

Yes, we are 2 separate entities.

ONLY if you present legal DPOA paperwork.

ONLY if you present Notarized Consent for Medical Treatment paperwork or guardianship paperwork.

Yes, this helps prevent an infection. Please do not use any lotions or perfumes before your surgery.
